Objawy skórne dotyczą ok. 90% chorych na kiłę.
Brak wysypki bądź jest obecność nie potwierdza ani nie wyklucza jednak zakażenia. W przypadku podejrzenia kiły należy wykonać odpowiednie badania diagnostyczne w laboratorium. Badania te wykonuje się z krwi. Wstępna diagnostyka obejmuje wykonanie tzw. testu VRDL lub USR. W przypadku pozytywnego testu, poszerza się diagnostykę i rozpoczyna leczenie.
