Approved indications for the use of duloxetine may include depression, generalized anxiety disorder and neuropathic pain in the course of diabetes.
However, antidepressants are sometimes used off-label (outside of the indications specified by the manufacturer in the leaflet) to reduce some of the symptoms of IBS. Their possible efficacy in this indication is due to interactions in the brain-gut axis and the fact that patients with IBS are more likely to have overlapping emotional disorders, including mood disorders.
They can also exert a certain analgesic effect in the case of pain or discomfort in the abdomen. Antidepressants are generally included when there is no improvement in the treatment of the predominant symptoms of pain and/or bloating - ultimately it is up to the doctor to choose the most appropriate treatment.
