In stress urinary incontinence, it is important to strengthen the Kegel muscles, because passing urine during physical exertion, coughing or sneezing is in this case caused by weakening of the pelvic floor muscles. It is recommended to reduce weight (if it is excessive), stop heavy physical work and smoking. Pessaries are also used - but mainly in those patients who do not want or can not undergo surgery. The surgical procedure involves the application of a special suburethral tape. Pharmacological treatment may include local estrogen therapy if the complaints are related to menopause and changes in the vulva and vagina are found. Sometimes duloxetine is used, which does not cure urinary incontinence, but reduces the symptoms of stress urinary incontinence and urgency. Solifenacin (the active ingredient in Silamil) is a medicine used in the pharmacotherapy of overactive bladder and has no indication for the treatment of stress urinary incontinence. Interdisciplinary guidelines of the Polish Urogynecological Society regarding the diagnosis and treatment of stress urinary incontinence in women do not contain recommendations for the use of solifenacin in this disease entity.
