Yes, they can be smeared with Bepanthen ointment, but you should not open the labia majora to introduce the cream there. In such young children, it is important to thoroughly clean (preferably with warm water) the area under the diaper in the direction from the pubic mound to the bottom each diaper change, so as not to transfer microbes from the anus to the urethra and vagina. The labia majora should be gently opened during washing to clean them of urine debris, poop and cream.
