The complex dual mechanism of action of tapentadol (the active substance in Palexia), i.e. agonistic effect on the opioid receptor μ (MOR) and inhibition of norepinephrine reuptake (NRI), contributes to its good efficacy and a lower incidence of opioid-specific side effects. The main indication for the use of Palexia is the relief of moderate to severe, acute pain. Due to its long duration of action, it is used in the case of chronic pain, also with a neuropathic component (e.g. in peripheral diabetic neuropathy). Palexia is indicated for patients with a history of bothersome nausea and vomiting after other opioid analgesics (e.g. Transtec).
