To @Ewa: Excuse me, you are right - I meant Encorton - a drug with prednisone. Cortiment and Entocort have the same active substance, they come in different doses, Cortiment are prolonged-release tablets, Entocort - hard capsules with prolonged valicity, therefore they can not be issued as substitutes. Thank you. I understand, so there must be a new recipe? Just one more thing, is ENTOCORT just as safe as CORTIMENT MMX? I mean, does it, like CORTIMENT MMX, work locally?

Thank you. From what I realized, Cortiment MMX and ENTOCORT have the same active substance - budesonide. Except that Cortiment MX comes in a dose of 9 mg, and ENTOCORT is a dose of 3 mg. So isn't it the same active substance? Do I need a new prescription if both Cortiment MMX and ENTOCORT contain budesonide?

TO @Ewa: The pharmacist cannot make such a change, a new prescription will be required. These drugs contain other active substances, differ in action and should not be considered as substitutes.

Can I ask for a replacement for Entocort at the pharmacy instead of Cortiment MMX?

The gastroenterologist prescribed me the steroid Entocort for eosinophilic esophagitis. As a result of a wide diagnosis, it was established that the disease is also accompanied by narrowing of the esophagus. Will this drug outside of EZP cure this narrowing of the esophagus that has arisen as a result of this disease? Will surgery be necessary?

To @Ewelina: In Poland, a drug containing budesonide in the form of orodispersible tablets is registered under the name Jorveza - it is an orphan drug, with indications for the treatment of eosinophilic esophagitis. There are no preparations with fluticosne available in Poland with an indication for the treatment of EoE. Under what names are drugs for eosinophilic esophagitis available in Poland? Are steroids such as fluticasone and budesonide available as a suspension for the treatment of EZP?

For @Karol: Steroids (mainly fluticasone and budesonide) administered topically into the esophagus are used to treat EoE. The use of steroid tablets or injections is usually not recommended.

For @Ewelina: Other medicines used to treat EoE can only be used under medical supervision, i.e. they are not available without a prescription.

I have already introduced a diet. I mean OTC drugs. I understand that nothing without a prescription can not be bought on EoE?

For @Ewelina: In addition to over-the-counter PPIs, an elimination diet can be introduced: - elimination of 6 food allergens (milk, gluten, eggs, soy, nuts, fish) gives remission of the disease in up to 3/4 of patients, - elimination of 4 allergens gives remission in about 50% of patients, - while the elimination of 2 allergens (milk and gluten) gives remission in 40% of patients. The introduction of a diet can be consulted with a nutritionist.

Of course, only for a visit to a gastroenterologist I have to wait almost 2 months. Hence my question whether I can buy something in the pharmacy without a prescription, which I could take until the visit to the doctor, to at least slightly alleviate the ailments?

To @Ewelina: With the disease, you should go to a specialist who, based on the symptoms and previous treatment, will select the most effective therapy.

Thank you. Is it possible to buy in a pharmacy any over-the-counter drug for eosinophilic esophagitis if over-the-counter PPI drugs do not help?

To @Ewelina: Unfortunately, we still have not received a response on Jorveza. According to the recommendations of an international group of experts from 2017, an elimination diet, corticosteroids or PPIs (proton pump inhibitors) are used to treat eosinophilic esophagitis. The studies showed that antihistamines (e.g. Clemastinum Hasco) had no effect on the symptoms and eosinophilia of the oesophagus.

According to current medical knowledge, the treatment of Eosin esophagitis includes dietary, pharmacological treatment and, in the case of esophageal stenosis, endoscopic expansion of the esophagus. During therapy, omeprazole is used - a substance belonging to the group of proton pump inhibitors. Available both in over-the-counter preparations, e.g. https://www.gdziepolek.pl/produkty/11808/bioprazol-bio-max-kapsulki/apteki, https://www.gdziepolek.pl/produkty/87996/helicid-control-kapsulki/apteki, as well as on prescription, e.g. https://www.gdziepolek.pl/produkty/34578/helicid-20-kapsulki/apteki, https://www.gdziepolek.pl/produkty/33652/goprazol-20-kapsulki/apteki. According to currently available knowledge, the drugs that are used in Eosine esophagitis, which are available only on prescription, are corticosteroids administered topically into the esophagus, e.g. in the form of syrup. The recommended substance is fluticasone - in Poland there is no such form of the drug on the market. Available studies also describe the use of systemic glucocorticoid - budesonide. Available preparations with this substance in Poland are, for example, Entocort https://www.gdziepolek.pl/produkty/27305/entocort-kapsulki-o-przedluzonym-uwalnianiu/apteki, Cortiment MMX https://www.gdziepolek.pl/produkty/87981/cortimentmmx-tabletki-o-przedluzonym-uwalnianiu/apteki. According to available sources, local action and administration of the above-mentioned substances in the form of, for example, syrups, carries fewer side effects than systemic therapy. In the literature, cases of administration of preparations containing the above substances are described, which are available in a form for nebulization, orally in the form of a specially prepared suspension. However, this is an application that should be done strictly on the recommendation and under the supervision of a doctor.
