Can I replace vaginal tablets with Vagif with estradiol cream for absorption through the skin for several months?

Good morning, Since December 2021 I have been taking Vagifem vaginal tablets, I am running out of stock of tablets, currently I do not live in Poland, unfortunately there is no Vagifem on sale here, I managed to buy a cream with estradiol for absorption through the skin. Can I replace vaginal pills with Vagif with this cream for a few months, and then after returning to Polish start taking Vagif again, or will I feel any side effects of such a replacement?

Unfortunately, I do not know what cream you bought, how it is supposed to work exactly and what are the indications for its use, so it is difficult for me to compare both products. However, the short description shows that it is not the equivalent of Vagifem and that it should not be used interchangeably without consulting a doctor. Vagifem is used to relieve the symptoms of vaginal menopause ('atrophic vaginitis'), which include dryness and the associated feeling of irritation. From Vagifem tablets, estradiol is released into the vaginal environment to act where there is a deficiency of its activity. The profile of Vagifem indicates very low systemic absorption of estradiol during treatment - it is therefore a hormonally active drug only locally. If you have bought a product that is to be absorbed through the skin, this effect will not be local, but most likely systemic. The hormone will penetrate into the blood and act on the whole body. It should be noted that if you have a preserved uterus, the use of estrogen alone systemically increases the risk of excessive thickening of the endometrium (endometrial hyperplasia) and malignant neoplasm of the endometrium (endometrial cancer). Therefore, preparations with estrogen alone in hormone replacement therapy for women with a preserved uterus are combined with progesterone, which reduces the risk of endometrial hyperplasia.
