Blood can come from a hemorrhoid or other lesion, if during the application of the suppository there was irritation of this place. On the other hand, suppositories used in the therapy of hemorrhoids are not designed to cause bleeding and "by themselves" should not work like that - on the contrary, many of them have components that soothe and seal blood vessels.
