Usually, if you miss a tablet at week 3 (i.e. if you have doubts about the 19th tablet, this applies to you), it is recommended to skip the placebo tablets and go straight to the active tablets from the next package.
So I should take a placebo pill and then, as always, start another blister? Will the effectiveness of contraception then be preserved?

It is difficult to answer this question - I have not seen the blister, nor do I know when this damage occurred. In case of any doubts about the effectiveness of a given tablet, it is better to take the tablet from the additional blister instead and continue using the remaining tablets normally.
