In this case, it is possible to purchase only with a prescription issued by a doctor for a specific patient. On one paper prescription, the doctor can write out the amount of tablets, which is enough for the patient for 120 days of treatment. Assuming that the patient will use 1 tablet of Euthyrox per day - 2 packs of 50 tablets or 1 pack of 100 tablets. Since the Volunteer Assistance Centre is not an entity performing medical activity (e.g. a POZ clinic - a Primary Health Care clinic), it is not possible to issue a demand. Demand is a form issued, for example, by a doctor or dentist for medicines that can be used in a given facility - e.g. for treatments. The document must contain, m.in, the data of the medical facility - this is regulated by the Pharmaceutical Law. Your Centre, even if it had, for example, a contract with a medical facility in Ukraine, cannot meet such a demand in Poland. To be able to meet the demand, it is necessary to m.in tax identification number (NIP). However, it is possible to implement Ukrainian prescriptions in Poland. Moreover, the pharmacist can issue a prescription to a patient who came from Ukraine directly at the pharmacy.
