If and when to increase the dose of Toujeo?

The previously used dose of 35j Toujeo suddenly ceased to be sufficient. Increase in body weight in recent weeks - from 80 to 84 kg (strength training). The question is whether it is worth increasing the dose? What conversion rate? I transferred the weight from cardio to strength training - 15 min cardio and 50 min strength 3-4 times a week. Age 31 years. Sugars cosmos for the last 7 days plus additional inflammation of the sinuses. Please help.

It is a base insulin, so it is difficult to say whether you need a change in the dosage of the base, since you can not see the entire glycemic profile. This question will be best answered by your diabetologist. The dose of insulin is determined on the basis of the demand for kg of body weight / 24 h, but it is also important m.in insulin sensitivity (the so-called converter - i.e. if 1 j of insulin reduces your sugar level) or seniority with the disease. Based on the information about the increase in body weight by 4 kg, I am not able to give a clear answer and it is worth discussing it with your doctor - to say that you exercise strength and to what extent, in your opinion, you should increase the dose of basal insulin and whether you should increase it at all. Sometimes in the patient it is enough to change the base by minimum values of 1-2 units, and sometimes (especially during infection) it is necessary to temporarily increase the base by e.g. 30%. It is worth observing sugars (I do not know whether you use the monitoring system or only a glucometer) and consider whether the really high glycemia (we do not know what and how long they last) are caused by too low dose of basal insulin or maybe intraprandial insulin (if it is used, but I assume that yes and that we are talking about type 1 diabetes, and not, for example, about type 2 diabetes). In addition, the last information is very important - about the inflammation of the sinuses, which can also be the cause (if not the main cause) of difficulties with controlling glycemia and the need to administer many corrective doses. It may happen that it is necessary to temporarily increase slightly and basal and intraprandial insulin. Of course, you must then take this into account and, for example, after increasing the base, remember to measure sugar also at night or take into account these changes in insulin dosage when planning the effort - after some time insulin sensitivity may return and there will be a greater risk of hypoglycemia.
