How can I fill prescriptions from other EU countries (Germany) in Poland or elsewhere?

A foreign prescription can be filled at any pharmacy. It is not possible to fill a prescription via the website - prescription drugs can only be obtained from stationary pharmacies.
The prescription must have the following data:
patient information: patient's name(s), patient's date of birth;
information about the issuer: name(s), surname and qualifications (professional title) of the issuer, contact details of the issuer (e-mail address or telephone or fax number with international prefix);
the address of the place where the health service is provided (e.g. the address of a clinic or doctor's office) with the country or its abbreviation;
information about the medicine: the international name of the medicine or, in exceptional cases, if the person issuing the prescription considers it necessary - the trade name - in this case, the reasons for using the proper name, form (e.g. tablets), dose and quantity of the medicine, dosage method are briefly stated on the prescription;
the date of the prescription;
handwritten signature of the issuing person.
A handwritten prescription must be legible to the pharmacist.
