My grandmother is diabetic type 2, she was given fresubin 2kcal instead of fresubin db. I don't know the glycemic index of 2kcal. Knowing that she only takes metformin, do you think it's very serious for her blood sugar?

I would recommend that you consult your doctor about this, and if you need to use it - you should do it carefully, monitoring your blood glucose levels. Fresubin DB is a better solution for people with type 2 diabetes due to its low glycemic index and composition adapted for diabetics, it allows for better glycemic control.
