If I don't have a prescription, can I stop taking Xarelto for 5 days?

From 27.04 I use Xarelto 15mg I have been prescribed 28 only tablets, but the next visit to the Hematologist I have only 30.05, which is 5 days without tablets. Can I calmly wait these 5 days without taking the drug or, for example, can I maintain the treatment of Vessel Due F (which I still have 25 tablets left from another doctor). Or should I ask for another doctor's prescription as so, should this sea medicine be prescribed by a primary care doctor? Thank you in advance for your reply

Do not stop treatment or replace Xarelto with Vessel Due F. A new prescription can be written by your GP based on the documentation from the haematologist. If you are not able to consult your doctor, you can use the teleadvice here: https://www.gdziepolek.pl/telekonsultacja
