I have insulin resistance and IPMN cysts on my pancreas. I was given metformin by the doctor (I take 2 tablets of 750 in the evening), recently the diabetologist gave me Rybelsus, because I still had high sugar of about 130. I read that this drug can cause acute pancreatitis. I have a question if I can take it, or will it be safer to increase metformin?

The decision to use Rybelsus in your situation should be made individually after consultation with your attending physician. This decision should be made together with the patient after a detailed discussion of the potential benefits and risks of Rybelsus in the context of the clinical condition, test results and patient preferences. It is difficult for me to suggest a specific method of treatment based on such a short description.
As a pharmacist, I can only give you general information:
If Metformin 1500 mg per day does not provide sufficient glycemic control, your doctor may consider adding Rybelsus. However, individual risks and benefits must be considered.
Rybelsus, a GLP-1 receptor agonist, can increase the activity of pancreatic enzymes and potentially lead to acute pancreatitis, especially in patients with pancreatic diseases such as IPMN cysts. Pancreatic function parameters should be closely monitored. This does not mean that it cannot be absolutely used in such a case.
